What is body composition?
Body composition describes what your body is made of, not how much it weighs. A review in Contrast Media & Molecular Imaging says the human body comprises more than thirty measurable components, and that a direct measurement of them in a living person is currently not possible. Indirect methods and models are used instead.1
The model used most often in clinics and population studies splits the body in two: fat mass and fat-free mass. Fat-free mass is everything else, including skeletal muscle, internal organs, bone mineral and body water.1 A body fat percentage is fat mass divided by total body weight, so it depends on the model behind it.
| Model | Parts | Where it is used |
|---|---|---|
| Two compartments | Fat mass and fat-free mass.1 | The most common model in clinical practice and epidemiology.1 |
| Three compartments | Bone mineral, fat and lean soft tissue.1 | What a whole-body DXA scan reports.1 |
| Four compartments | Adds measured body water to bone, fat and lean mass.4 | A laboratory model used as the reference in a 15-device study, built from air displacement, DXA and bioimpedance spectroscopy.4 |
| Five levels | Atomic, molecular, cellular, tissue-system and whole body.2 | A research framework that organizes body composition studies.2 |
The compartment models above are summaries of the cited reviews and studies.
Most readers come here for the body fat number. The companion guide on body fat percentage covers ranges. This page covers what lies behind the number and how reliable each method is.
Why weight alone is not enough
Weight adds fat, muscle, bone and water into one number. The CDC says body mass index, or BMI, is a calculated measure of weight in kilograms divided by the square of height in meters. It also says BMI does not directly measure body fat, does not distinguish between fat, muscle and bone mass, and does not show what type of fat a person has or where it sits. For individuals, BMI should be considered with other factors.3
Worked example: same weight, same BMI, different makeup
Two made-up adults both weigh 180 lb and stand 70 in tall. 180 lb is 81.65 kg and 70 in is 1.778 m, so BMI is 81.65 ÷ (1.778 × 1.778) = 81.65 ÷ 3.161 = 25.8 for both. Now give Person A a body fat percentage of 20% and Person B 30%. Person A carries 180 × 0.20 = 36 lb of fat and 144 lb of fat-free mass. Person B carries 180 × 0.30 = 54 lb of fat and 126 lb of fat-free mass. The difference is 18 lb of fat and 18 lb of fat-free mass, with identical weight and BMI.
These percentages are invented to show the arithmetic. The point is the one the CDC makes: a ratio of weight to height cannot see inside the body.3 For the BMI categories themselves, see the adult BMI chart.
How do the methods compare?
A review of methods in the Journal of Diabetes Science and Technology says body composition assessments vary in precision and in the tissue they target, and that no single method is universally recommended in people with obesity. Each has benefits and drawbacks.10 The table lists what the papers read for this guide say about each.
| Method | What it estimates | What studies report | Main limit |
|---|---|---|---|
| Bioelectrical impedance (home scale) | Body water and fat-free mass from electrical resistance, then fat.1 | Repeat readings differed by 0.0 to 0.49 percent. Standard error against a four-compartment model was 3.1 to 7.5 percent.4 | Water intake shifts the estimate. Models differ by device.412 |
| DXA scan | Bone mineral, fat and lean soft tissue.1 | Precision is excellent, about 1% coefficient of variation for whole-body bone in young healthy people. Accuracy can be problematic.5 | Cost, expertise, radiation, and results differ between manufacturers.51 |
| Air displacement (Bod Pod) | Body density, then fat percentage.6 | Against DXA it overestimated body fat in thin people and underestimated it in heavier people, by up to 13 points in some underweight participants.6 | Agreement depends on body size.6 |
| Skinfold calipers | Fat under the skin at set sites, fed into an equation.7 | Against DXA in 50 college students, total error was 7.9 to 10.9 body fat points for two calipers, and individual limits of agreement were up to 8.6 points.7 | Depends on the person measuring and the equation. |
| Tape measure (Navy-style circumferences) | Body fat from neck, waist and hip measurements.8 | In a validation of overfat Navy members, false positives for excess body fat ran 6.8% to 18%, and the authors urged caution for individual decisions.8 In 21 male cadets, a military circumference equation underestimated body fat percent and fat mass changes against air displacement.9 | Prediction error is greater at extreme values.8 |
| BMI | Weight relative to height only.3 | Does not distinguish fat, muscle and bone.3 | No direct body fat information. |
Findings come from different studies with different groups and references, so the rows are not a ranked comparison. Several reference methods are themselves imperfect.5
Two more points from the papers. Laskey’s review concludes that DXA cannot be regarded as a gold standard for body composition, although its use has been growing,5 while a newer overview calls it the current reference method for assessing body composition because it provides accurate estimates of bone mineral, fat and lean soft tissue.1 Both statements are in the literature. The practical meaning: a DXA scan is the better-known reference, not an infallible one.
On safety, the overview gives effective doses of 1 to 7 microsieverts for DXA and says no more than two body scans a year are currently advised.1 MedlinePlus, describing the bone density version of the test, says you receive more radiation from a chest x-ray than a DEXA scan.13
Bioelectrical impedance: the basics
Bioelectrical impedance analysis, or BIA, is the method inside most body composition scales. It sends a low-voltage electric current through the body and measures how much the body opposes it. The overview says the transit of the current depends on body composition characteristics, and that bioimpedance varies with tissue composition and with the frequency of the current.1
The ESPEN working group review explains the use: BIA allows the determination of fat-free mass and total body water in people without significant fluid and electrolyte abnormalities, when appropriate population-, age- or pathology-specific equations and established procedures are used.14 The overview adds that total body water is approximately 73% of fat-free mass in healthy individuals.1 Fat mass is what is left after the equation takes fat-free mass out.
| Type | How it works | Detail |
|---|---|---|
| Single-frequency, hand-to-foot | Generally 50 kHz between surface electrodes on hand and foot.1 | The classic clinical layout. |
| Foot-to-foot or hand-to-hand | Electrodes at the feet or at the hands only.1 | Foot-to-foot gave the lowest fat-free mass values in overweight and obese people, also against DXA.1 |
| Multi-frequency | Uses several frequencies, from 1 kHz up to 500 kHz.1 | Low frequencies stay outside cells. Frequencies above 50 kHz pass through cell membranes. Above 100 kHz does not improve accuracy.1 |
| Bioimpedance spectroscopy | A wider sweep of frequencies.1 | It does not need statistically derived, population-specific equations and can tell extracellular from intracellular water.1 |
Summarized from Marra and colleagues (2019). Consumer scales rarely state which model or equation they use.

What goes wrong with impedance
- Hydration. In 39 adults, drinking 2 liters of water raised measured body fat percentage by 1.3 to 2.6 points.12
- Equations. Equations are built on a sample, so age and ethnicity can change the result, and the assumption that hydration stays constant may fail.11
- Muscle and visceral fat. Over a 12-month weight loss program, impedance matched DXA on fat lost but read skeletal muscle 1 to 2 kg low, and overestimated visceral fat against MRI.15
- Standardization. The accuracy of BIA is reduced when specific predictive equations and standardized measurement protocols are not used.1
A note on implanted devices. A benchtop and simulation study did not recommend consumer bioimpedance devices for people with cardiac implantable electronic devices because of possible interference.16 A separate study of 200 patients found none.17 Chutex says its scale is not for people with a pacemaker.18 If you have one, ask your cardiology team.
Choosing a method for your question
No method answers every question, so match the tool to the decision. This table is a set of criteria, not a ranking.
| Your question | Useful tools | Why |
|---|---|---|
| Am I changing over weeks? | The same device, same conditions, repeated. | Precision under standardized conditions was high in the 15-device study.4 |
| What is my fat percentage today? | A reference test such as DXA or a lab model, read with its limits in mind. | Single readings from home scales varied widely against a lab model.4 |
| Is my waistline changing? | A tape measure at the same spot. | Circumference equations are rough for individuals, but the tape itself is simple.8 |
| Do I have a health problem? | A clinician. | A health question calls for clinical testing. Clinical populations are a different case from healthy home users.19 |
The ASPEN guidelines made no recommendation for BIA in clinical populations because data were limited and manufacturers keep their equations proprietary.19
- Choose one method. Use the same scale or the same tape for every check.
- Fix the conditions. Same time of day, similar food and drink beforehand, same clothing and place.
- Record the average. Write down several readings and look at the weekly average, not single days.
- Cross-check with a second signal. Compare with a waist measurement, how clothes fit and your strength in daily life.
- Ask for a reference test if it matters. For a health decision, talk to a clinician about the right test.
For specific measures, see visceral fat, muscle mass, body water percentage, waist-to-hip ratio and how to measure body fat at home. To compare a scale with a scan, read DEXA scan vs smart scale. If you want the buying angle, the guide to the best smart scales for body composition lists criteria.
Frequently asked questions
What is body composition?
It is the makeup of the body, most often split into fat mass and fat-free mass. Fat-free mass includes muscle, organs, bone and water. A direct measurement in a living person is not currently possible, so methods estimate it.
What is bioelectrical impedance analysis?
BIA sends a low-voltage current through the body and measures how much the body opposes it. Equations then convert the result into estimates of total body water and fat-free mass. Accuracy depends on the device, the equation and conditions such as hydration.
How accurate is bioelectrical impedance for body fat?
In a study of 15 devices, repeat readings differed by 0.0 to 0.49 percent, but the standard error against a four-compartment lab model was 3.1 to 7.5 percent. Use it to follow trends.
Is BIA as good as a DEXA scan?
Not for single readings in the studies read here. DXA is widely used as a reference, though one review argues it should not be called a gold standard. See the guide that compares a DEXA scan with a smart scale.
Why is BMI not enough?
The CDC says BMI does not distinguish between fat, muscle and bone mass and does not show where fat sits. Two people can share a BMI and carry different amounts of fat.
What does body composition analysis involve?
It can be as simple as a tape measure or a bathroom scale, or as involved as a DXA scan or lab model. Methods differ in precision, cost and what they target.
Does drinking water change a body composition reading?
Yes. In 39 adults, drinking 2 liters of water raised the measured body fat percentage by 1.3 to 2.6 points. Compare readings taken under the same conditions.
Is it safe to use an impedance scale with a pacemaker?
Evidence is mixed: one simulation and benchtop study did not recommend consumer devices for people with implanted cardiac devices, and Chutex says Vita is not for them. Ask your cardiology team first.
